Bring the required paperwork

Bring any forms provided by the school, athletic program, or camp, along with a medication list, relevant medical records, and immunization information if requested.

Complete the health evaluation

The provider can review health history, perform the appropriate physical examination, and discuss any symptoms, injuries, or health concerns that may affect participation.

Review clearance and next steps

When appropriate, forms can be completed after the evaluation. If a concern needs more assessment, Lake Med can explain what follow-up may be needed before clearance.

What should I bring to the physical?

Bring the required school, sports, or camp form, a current medication list, relevant medical records, and any immunization documentation requested by the organization.

Is a sports physical the same as a regular wellness visit?

Not necessarily. A sports physical is focused on participation and activity readiness, while a comprehensive wellness visit may cover a broader range of preventive and ongoing health needs.

Can Lake Med complete my form during the visit?

Forms can generally be completed when the required evaluation is finished and the provider has enough information to make an appropriate determination. Additional records or follow-up may sometimes be needed.

What happens if a health concern is found?

A concern does not always prevent participation, but additional evaluation, testing, treatment, or specialist clearance may be recommended depending on the finding and the activity involved.
