LakeMed does not currently accept patient intake, appointment, membership, or medical-information submissions through forms on lakemed.us. If you call or email LakeMed, we receive the information you choose to provide through that communication.
For your privacy, do not send medical information, medical records, diagnoses, medications, insurance details, or other sensitive health information through ordinary website contact methods or standard email. Use LakeMed's secure patient portal for communications involving your care or medical record, or call the office. Call 911 for an emergency.
